This form is not to register into the program. It is designed to help us gather
some information about you so we may place you in a tribe that best fits you and
your child. This information will be sent to the Federation Organizer, please allow
a few days for a response. Along with your tribal assignment, the Federation Organizer
will give you instructions on how to register into the program.
